In order to participate in programs offered by the City of Hermosa Beach Community Resources Dept., you must agree to the following liability waiver.

I, the participant, parent or guardian, do hereby agree to participate and/or allow the individual(s) named herein to participate in the aforementioned activity(ies) and further agree to indemnify and hold harmless the City of Hermosa Beach, its agents and employees from harm, accidents, personal injury or property damage which may be suffered by the aforementioned individual(s) arising out of, or in any way connected with, the participation of the activity.

If you use HB Connect you must actively agree to this waiver in order to complete your registration. If you do not agree, you will not be able to register for any activity. If you register by fax, mail or in person, a signed waiver will be required.
